Department of Cardio Thoracic Surgery in Madurai Medical College and Government Rajaji Hospital, Madurai started functioning in the year 1965 with one Reader and two Assistant Professors. From that period, the department was carrying out the Closed Heart Surgeries and other Thoracic Surgeries including Lung and Oesophageal Surgeries. The quantum of work in Closed Heart Surgeries like Closed Mitral Valvotomy and Patent Ductus Arteriosus Surgeries were more in the initial period of our department. Gradually the programme of Open Heart Surgery started in the year 1984 with the sanction of one Additional Professor and two more Assistant Professors. From the day of inception of Open Heart Surgical Programme, the quantum of work in Open Heart Surgery is gradually increasing up to date.  

To start with Congenital Heart Disease like Atrial Septal Defect, Ventricular Septal Defect, etc., and Valvular Heart Diseases like Mitral Regurgitation and Aortic Regurgitation were dealt with and Valve Replacements were done on regular basis. We are making use of Artificial valves for the replacements of Mitral Valve and Aortic Valve. These patients are mostly all in the category of Rheumatic Etiology. Among the congenital cases, Atrial Septal Defect is occupying a large volume. We prefer Starr Edwards Ball valves in the Mitral position and Disc valves in the Aortic position. The morbidity and mortality is as equal to any other international center, even though we deal with this type of Open Heart Surgery patients in our post-operative ward with the least availability of post-operative care equipments.  

From the year 1997, we started the programme of Coronary Bypass Surgery and we are now doing regular coronary intervention surgical procedures and we are in a position to deal with all types of conduits in the coronary bypass surgery. We are making use of saphenous veins, radial arteries and internal mammary arteries, both in on pump and off pump bypass surgeries. Regarding the Thoracic Surgical work, we do the required surgeries for Thoracic Tumours, Lung Cancer, Oesophageal Surgeries and surgeries connected with Chest Injuries etc. etc., our department activity is extended to other departments which are in need of our help to deal with the surgical procedures conducted in other theatres like Trauma Ward and Ortho department, etc. Thoracoscopic surgical procedures and the minimal invasive thoracic procedures are also conducted in our department.  

Our department is having the Post-graduate teaching programme and every year Two (02) candidates are admitted since 2001 and from 2019, Four (04) candidate are admitted for the Super speciality Postgraduate course of M.Ch. (Cardio Thoracic Surgery) Our department is giving special Cardio Thoracic training to M.S. (General Surgery) postgraduate students and regular teaching programme is being conducted to the Undergraduates. Our department is the tertiary care /Referral center for all cardiac and lung surgical cases from south zone of Tamilnadu. From the date of commencement of Open Heart Surgery, the quantum of surgical excise is increasing till date. 

AIM & OBJECTIVE:
The syllabus for M.Ch. (Cardiovascular and Thoracic Surgery) Course comprehensively covers all subjects related to Cardiovascular and Thoracic Surgery during the 3-year study period. The student will gain exposure to all facets of the specialty and develop adequate knowledge and skills to treat patients competently upon acquiring the degree.

Theory Syllabus includes:

  • Applied Anatomy and Developmental Anatomy of the chest wall, diaphragm; Pleura, Lungs, Mediastinum, Oesophagus, Heart, Pericardium, Great Vessels, and branches; Congenital cardiovascular and thoracic anomalies.

  • Applied Physiology: Respiration, Pulmonary function tests, Blood Pressure, Cardiac cycle, Cardiac output, Heart sounds and murmurs, Extracorporeal circulation, Hypothermia, Cardiac metabolism, Acid-base balance, Fluid and electrolyte balance, Oesophageal physiology, Gastro-oesophageal reflux.

  • Applied Pathology: Thoracic injuries, Chest wall tumours, Emphysema, Pleural tumours, Pulmonary suppuration, Congenital anomalies of great vessels, Oesophageal conditions (benign strictures, cancer, achalasia, oesophagitis), Diaphragmatic hernia, Cardiac tumours, Pulmonary embolism.

  • Applied Bacteriology: Thoracic and pulmonary infections, bacterial and fungal endocarditis, infections during open heart surgery, nosocomial infection/HIV in cardiac and pulmonary disease.

  • Clinical examination and management of chest injuries, deformities, pleural infections, pulmonary tuberculosis, lung abscesses, bronchiectasis, lung cysts, mediastinal tumours, and congenital oesophageal anomalies.

  • Diseases of the pericardium, myocardium, endocardium, rheumatic heart disease, conduction system diseases, coronary diseases, congenital cyanotic and acyanotic heart diseases, aneurysm of great vessels, detailed treatment knowledge of cardiac, vascular, and thoracic surgery problems.

  • Knowledge of extracorporeal circulation, recent advances like ECMO, heart/lung transplant, robotic surgery, genetic engineering in cardiac diseases, minimally invasive and video-assisted thoracic surgery.

Clinical Training:
Students receive hands-on training in the parent department and related departments through structured postings:

  • Cardiology – 1 month

  • Vascular Surgery – 1 month

  • Advanced procedure institutions – 1 month

During the 2nd year, students can take special postings at reputed centers to learn advanced techniques without affecting their parent department duties.

Skill Training Requirements:
Surgical skills are developed progressively over 3 years by assisting or performing surgeries under supervision:

  • Year 1: Assist in valve replacements, congenital heart surgery, coronary artery bypass grafting, lung resection, decortication, pericardectomy; Perform sternotomy, pleurocentesis, pericardiocentesis, intercostal drainage, thoracotomy, saphenous vein harvesting.

  • Year 2: Assist in beating heart CABG, atrial septal defect repair, minimally invasive cardiac surgery, video-assisted thoracic surgery; Perform mitral valve replacement, proximal coronary anastomosis, bronchoscopy, traumatic lung injury repair.

  • Year 3: Assist complex congenital heart surgeries, aortic/double valve replacements, Bentall procedure, multiple valve replacements, redo cardiac surgeries, vascular repair; Perform distal coronary anastomosis, lung resection, decortication.

Teaching Methodology:

  • Weekly planning sessions with faculty and students

  • OP clinics 1-3 days/week

  • Ward rounds 3 days/week

  • Per-operative teaching 2 days/week

  • Classroom lectures thrice weekly

  • Weekly video sessions, journal reading, symposiums

  • Guest lectures bi-monthly

  • Monthly CME programs

Research Work:

  • Training in clinical and laboratory research in cardiovascular and thoracic surgery

  • Ability to frame research questions, plan studies, collect and evaluate data

  • Reporting results in papers and presentations

  • Organizing and submitting a research paper at course end

  • Mandatory Research Methodology workshop within first 6 months

Log Book:
Students maintain detailed records of training, surgical procedures assisted or performed, reviewed regularly by faculty and department head.

Competency Assessment:
Evaluation based on communication, commitment, innovation, documentation, case presentations, academic achievements – total 50 marks across six periodic assessments, plus a final viva and competency exam worth 100 marks.

Examinations:

  • Theory: Four papers covering basic sciences, thoracic and cardiovascular surgery, and recent advances.

  • Clinical: Long and short case exams, ward rounds, OSCE stations, viva voce, and log book assessment.

Reference Books & Journals:
Includes standard textbooks and journals like Sabiston, Kirklin, Indian Journal of Thoracic and Cardiovascular Surgery, The Annals of Thoracic Surgery, and others.
